Professional Learning Expense Claim Forms for Ottawa Carleton ETFO/FEEO Members

***IMPORTANT EXPENSE REIMBURSEMENT UPDATE***

 

There are 3 options for you to choose from for method of reimbursement:

1.  Cheque - Would you like the cheque to your home address or school via board mail?  Please be sure to indicate on the form

2.  E-transfer - **If you choose e-transfer the password will be PLPayment** Please be sure to indicate on the form

3.  Electronic Funds Transfer - Please complete the attached Electronic Funds Transfer Payment form found below.  Please send the completed forms, together with a blank cheque, and any applicable receipts via email to Margo Charles margo.charles@ocetfo.org

Please note, once the transfer has been completed and payment confirmed all documents will be shredded.  You will be registered to receive payment in this manner going forward.

*Electronic blank cheques can usually be created by logging into your bank account.
